Marketplace Application Checklist

Prepare household, income, coverage, and deadline records for a Marketplace application, then verify plan selection and activation.

Source checked

Use this checklist on paper or copy it into a private document. No account is needed to use the checklist, and this page does not collect or submit your information. It prepares you for an official application; it does not determine eligibility or enroll anyone. Source guidance was checked November 2, 2026. Write the coverage year beside every deadline.

1. Identify your application route

Record these items before gathering sensitive documents:

  • Coverage year: ______. State and county of residence: ______.
  • People requesting coverage: ______. Current coverage end dates: ______.
  • Official Marketplace website and contact route: ______.
  • Enrollment opportunity and deadline confirmed by that Marketplace: ______.
  • Desired effective date and any conditions attached to it: ______.

Use the official state directory. The correct destination can change by year; Oregon's 2027 application route, for example, differs from its 2026 coverage-information route. Choose the year you actually need. Official application options include online, phone, assisted, and paper routes. Record which route you will use and how you will retain confirmation.

2. Prepare the household record

Make a private list with each person's relationship to the tax filer, expected tax-dependent status, whether they need insurance, and existing coverage. Do not reduce the application to only the people seeking a new policy.

Follow the household instructions, including their exceptions. A tax household is not simply everyone at the same address. Gather identifying or immigration documents only as needed under official instructions, and keep actual identifiers out of a checklist you might share for help.

  • Expected tax filer and filing arrangement: ______.
  • Spouse and dependents to include: ______.
  • Household questions requiring an assister's answer: ______.
  • Existing coverage or employer offers for each person: ______.

3. Prepare a defensible income estimate

List expected income for the coverage year, not just last year's tax return. Gather recent pay records, business records, and documentation for other income categories. Use HealthCare.gov's income list and the ACA Income and MAGI Worksheet to identify countable amounts.

  • Expected annual household income and calculation date: ______.
  • Current monthly income if requested: ______.
  • Known changes, irregular earnings, and supporting assumptions: ______.
  • Uncertain items requiring tax or Marketplace assistance: ______.

Do not interpret missing paperwork as permission to invent an amount. Document a reasonable estimate and update it when relevant facts change.

4. Document a special enrollment event if needed

Outside the applicable annual window, check official special enrollment rules. Record the event date, previous coverage dates, requested evidence, and submission deadline. Different events have different conditions; a move or family change is not an unconditional right to any plan.

  • Event and date: ______.
  • Evidence requested in the eligibility notice: ______.
  • Document deadline, submission method, and confirmation: ______.

Keep the loss-of-coverage notice or other requested evidence. Do not assume the deadline for choosing a plan is also the deadline for sending every document.

5. Review results and select coverage

Read the complete eligibility notice. Record who qualifies for which program, any premium tax credit, any cost-sharing assistance, and unresolved verification requests. Use the plan comparison guide to check providers, drugs, premium, deductible, and spending limits.

  • Eligibility result by person: ______.
  • Selected plan name, identifier, network, and coverage year: ______.
  • Premium after approved assistance and payment instructions: ______.
  • Questions resolved before selection: ______.

Ordinary income-based cost-sharing reductions require an eligible Silver plan; special American Indian and Alaska Native protections follow different rules. Use your eligibility notice to identify the correct plan variation rather than comparing only a generic brochure.

6. Confirm activation and maintain the record

Follow the insurer's first-premium and activation instructions. Pay any required amount to the insurer by its deadline and retain the receipt. Confirm active coverage and its effective date before ending another policy.

  • Application and plan-selection confirmations saved: ______.
  • Required first payment received by insurer: ______.
  • Effective date verified with insurer: ______.
  • Outstanding document requests completed: ______.
  • Reminder to report relevant income, household, or coverage changes: ______.

If a step remains unresolved, write the responsible office and next contact date. Completing this worksheet is preparation; the official notices and insurer confirmation establish what happened to your application and coverage.
